Office Visit Copay Plans: These plans require co-pays (a flat dollar amount) for “in-network” diagnostic illness and injury doctor’s office visits. Preventative care services are covered at 100% when your employees see an in-network doctor. Prescription drug coverage is also included.

100% After Deductible Plans: This plan sets an out-of-pocket maximum, which is the most an employee will pay for a deductible each year. Once the out-of-pocket maximum is reached, this plan will pay 100% of covered medical expenses.

HSAs, HRAs, and FSAs: These three plans—Health Savings Plans (HSA), Health Reimbursement Arrangements (HRA), and Flexible Spending Accounts (FSA)—can be coupled with high-deductible health plans. These plans help your employees pay for their health care services with tax-advantaged funds.

Dental and Vision Insurance: You may add dental and/or vision insurance as a complement to a health plan or as stand-alone coverage.
